Loyalty Program: Terms and Conditions
Welcome to the Ivim Health LLC’s (hereinafter “Ivim Health”) Loyalty Program! By participating in the Loyalty Program you agree to these Loyalty Program Terms and Conditions, which also include Ivim Services LLC’s Terms and Conditions and Privacy Policy (“Loyalty Terms”).
The Ivim Loyalty Program (hereinafter “Loyalty Program”) is a loyalty program that provides incentives for Ivim Health Members to share Ivim Health’s wellness services to their community.
Eligibility
Participants must be age 18 years or older to participate and have an active monthly subscription membership with Ivim Health (“Members”). Only Members are eligible to participate in the Ivim Health’s Loyalty Program (“Loyalty Members”).
How It Works
When you sign up to participate in the Loyalty Program, you will receive a unique loyalty code (“Code”) that you may share. Each Code is unique, meaning a unique user may only use it once in which you will receive credit. For example, new patients will receive their first month of membership free when they sign up using a valid Code. Once someone uses your Code, you will earn points that you can use toward your own monthly membership fee.
For each new monthly membership that is activated using your Code, you will receive up to $74.99 toward your next monthly membership subscription cost. In other words, once redeemed, a membership reward will be applied to your account at your next membership renewal date. If your monthly membership is $74.99, you will not be charged for your monthly membership that month. If, however, your monthly membership is more than $74.99, you will still be required to pay any remaining balance between your monthly membership costs and the $74.99 membership reward. Regardless of how much you pay for your monthly membership, no overage will be carried over to count against or credit your next month’s balance.
You may redeem up to 12 membership rewards on a rolling 12-month basis. This means that at any given time, you can have up to 12 unapplied membership rewards (redeemed but not yet used) within the past 12 months. For example, after the reward is applied, i.e., used for a calendar month, a spot opens up within your 12-reward limit, allowing you to redeem another membership reward, provided you have enough points.
You can redeem membership rewards at any time, provided you have sufficient points and have not exceeded the redemption limit.
Loyalty Members may also receive points for other activities as Ivim Health may notify you about and update these Loyalty Terms. Activities and events to earn points can be added or removed from the Loyalty Program at any time. Values associated with these activities and events can be modified at any time.
Changes, Termination, and Cancellation
Ivim Health reserves the right to modify or terminate the Loyalty Program at any time, with or without notice. In the event of termination or cancellation, any unused points will remain valid for a 30-day period.
Abuse and Fraud Prevention
Accounts suspected of fraud or abuse may be suspended of terminated without notice and all rewards associated therewith will be forfeited.
